I was filling out the form and wondering which box at the bottom we should be selecting...do we need a new card, just a new written piece of documentation? does anybody know ?

For the new criminal reference check we need the last box from my understanding. This one gives us written documentation on our status and we are to send copies of this to Boards requesting for it, as well as to OCT. I believe we get two original copies so that one original can actually be sent to OCT with the other one being kept for our records. I hope this helped.
